All templates which are required when creating an accreditation and enrolement, can be found within the 'Downloadable Templates' section on the accreditation and enrolment.
Please download the template, fill out the required fields and then upload to the specifc area for the client or Local Autority to review.
Technical support for uploading or updating documents in the system can be sought from the platform's support team. However, note that the support team can't modify the content or provide advice on document specifics set by the Local Authority/ICB.
If the required template is not available, you can contact the Local Authority or Integrated Care Board (ICB) directly to request it. Ensure you provide all necessary details in your communication. For system-provided templates, it is the responsibility of the Local Authority or ICB to ensure the content is up-to-date.
