External referrers are able to track the progress of a submitted CHC referral and be notified via automated emails on key milestones throughout the assessment process.
Progress tracking dashboard
To see the dashboard of all the submitted CHC referrals, follow these simple steps:
Log in to SProc as a external referrer.
Within the CMS tab, click on the All tab.
All submitted referrals are visible here with their corresponding status.
Email notifications
There are four types of automated email notifications external referrers will recieve at key milestones:
Notification type | Trigger | Email contents |
Referral Status Changed | Referral status updates. For example, Pending QA to Eligible for Assessment. | Referral ID, Old Status, New Status. |
Assessor Assigned | Assessment created and assessor allocated. | Referral number, Date of allocation, Assessor name, Link to referral. |
Assessment Appointment | Assessment date, time and location scheduled or updated. | Referral number, Assessor name, date and time, Location. |
Assessment Decision | CHC eligibility decisions recorded. | Referral number, Decision outcome, Decision date, Eligibility from date, Current eligibility status. |
Team members will receive notifications for referrals submitted by colleagues and each email will include a direct link to view the referral in the system.
External referrers can manage thier own email notification preferences via the Manage Notifications tab.
Allowing them to independently toggle personal and team notifications on or off for 16 different notification types.
